Sadler’s Community Health Worker team is here to assist patients with barriers. Our CHW’s can answer questions that will help determine needs. CHW’s can connect patients to valuable resources like food banks, shelters, transportation assistance, and other basic needs while patients develop or restore self-sustaining solutions for these things. CHW’s can also help connect patients to their primary care providers, behavioral health specialists, or case managers.
